56 yr old man with a history of depression, self inflicted wound with knife in suicide attempt. Went to local hospital and had emergency throacotomy and pericardial effusion evacuated, no cardiac injury needed repair. Transferred to us, required multiple thoracic surgeries for pulmonary complications, infections and eventually died
